An Advance Health Care Directive is a legal document stating a specific individual who would like to name a specific person to make any and all medical decisions once they are not incapcitated. 

A guardianship is for a minor who has been left without someone to care for them. This is when someone is willing to step in and care for their needs. They will need to file a guardianship petition.

A Durable Power of Attorney is legal document that is  created when someone names a specific individual to pay their bills or receive income such as rent from rental properties.
